RESUMO

PURPOSE: Transarterial radioembolization (TARE) procedures treat liver tumors by injecting radioactive microspheres into the hepatic artery. Currently, there is a critical need to optimize TARE towards a personalized dosimetry approach. To this aim, we present a novel microsphere dosimetry (MIDOS) stochastic model to estimate the activity delivered to the tumor(s), normal liver, and lung. METHODS: MIDOS incorporates adult male/female liver computational phantoms with the hepatic arterial, hepatic portal venous, and hepatic venous vascular trees. Tumors can be placed in both models at user discretion. The perfusion of microspheres follows cluster patterns, and a Markov chain approach was applied to microsphere navigation, with the terminal location of microspheres determined to be in either normal hepatic parenchyma, hepatic tumor, or lung. A tumor uptake model was implemented to determine if microspheres get lodged in the tumor, and a probability was included in determining the shunt of microspheres to the lung. A sensitivity analysis of the model parameters was performed, and radiation segmentectomy/lobectomy procedures were simulated over a wide range of activity perfused. Then, the impact of using different microspheres, i.e., SIR-Sphere®, TheraSphere®, and QuiremSphere®, on the tumor-to-normal ratio (TNR), lung shunt fraction (LSF), and mean absorbed dose was analyzed. RESULTS: Highly vascularized tumors translated into increased TNR. Treatment results (TNR and LSF) were significantly more variable for microspheres with high particle load. In our scenarios with 1.5 GBq perfusion, TNR was maximum for TheraSphere® at calibration time in segmentectomy/lobar technique, for SIR-Sphere® at 1-3 days post-calibration, and regarding QuiremSphere® at 3 days post-calibration. CONCLUSION: This novel approach is a decisive step towards developing a personalized dosimetry framework for TARE. MIDOS assists in making clinical decisions in TARE treatment planning by assessing various delivery parameters and simulating different tumor uptakes. MIDOS offers evaluation of treatment outcomes, such as TNR and LSF, and quantitative scenario-specific decisions.

Purpose. Lymphopenia is a common side effect in patients treated with radiotherapy, potentially caused by direct cell killing of circulating lymphocytes in the blood. To investigate this hypothesis, a method to assess dose to circulating lymphocytes is needed.Methods. A stochastic model to simulate systemic blood flow in the human body was developed based on a previously designed compartment model. Blood dose was obtained by superimposing the spatiotemporal distribution of blood particles with a time-varying dose rate field, and used as a surrogate for dose to circulating lymphocytes. We discuss relevant theory on compartmental modeling and how to combine it with models of explicit organ vasculature.Results. A general workflow was established which can be used for any anatomical site. Stochastic compartments can be replaced by explicit models of organ vasculatures for improved spatial resolution, and tumor compartments can be dynamically assigned. Generating a patient-specific blood flow distribution takes about one minute, fast enough to investigate the effect of varying treatment parameters such as the dose rate. Furthermore, the anatomical structures contributing most to the overall blood dose can be identified, which could potentially be used for lymphocyte-sparing treatment planning.Conclusion. The ability to report the blood dose distribution during radiotherapy is imperative to test and act upon the current paradigm that radiation-induced lymphopenia is caused by direct cell killing of lymphocytes in the blood. We have built a general model that can do so for various treatment sites. The presented framework is publicly available athttp://github.com/mghro/hedos.

Objective. Phantoms of the International Commission on Radiological Protection provide a framework for standardized dosimetry. The modeling of internal blood vessels-essential to tracking circulating blood cells exposed during external beam radiotherapy and to account for radiopharmaceutical decays while still in blood circulation-is, however, limited to the major inter-organ arteries and veins. Intra-organ blood is accounted for only through the assignment of a homogeneous mixture of parenchyma and blood [single-region (SR) organs]. Our goal was to develop explicit dual-region (DR) models of intra-organ blood vasculature of the adult male brain (AMB) and adult female brain (AFB).Approach. A total of 4000 vessels were created amongst 26 vascular trees. The AMB and AFB models were then tetrahedralized for coupling to the PHITS radiation transport code. Absorbed fractions were computed for monoenergetic alpha particles, electrons, positrons, and photons for both decay sites within the blood vessels and for tissues outside these vessels. RadionuclideS-values were computed for 22 and 10 radionuclides commonly employed in radiopharmaceutical therapy and nuclear medicine diagnostic imaging, respectively.Main results. For radionuclide decays, values ofS(brain tissue ← brain blood) assessed in the traditional manner (SR) were higher than those computed using our DR models by factors of 1.92, 1.49, and 1.57 for therapeutic alpha-emitters, beta-emitters, and Auger electron-emitters, respectively in the AFB and by factors of 1.65, 1.37, and 1.42 for these same radionuclide categories in the AMB. Corresponding ratios of SR and DR values ofS(brain tissue ← brain blood) were 1.34 (AFB) and 1.26 (AMB) for four SPECT radionuclides, and were 1.32 (AFB) and 1.24 (AMB) for six common PET radionuclides.Significance. The methodology employed in this study can be explored in other organs of the body for proper accounting of blood self-dose for that fraction of the radiopharmaceutical still in general circulation.

Cuprate high-Tc superconductors are known for their intertwined interactions and the coexistence of competing orders. Uncovering experimental signatures of these interactions is often the first step in understanding their complex relations. A typical spectroscopic signature of the interaction between a discrete mode and a continuum of excitations is the Fano resonance/interference, characterized by the asymmetric light-scattering amplitude of the discrete mode as a function of the electromagnetic driving frequency. In this study, we report a new type of Fano resonance manifested by the nonlinear terahertz response of cuprate high-Tc superconductors, where we resolve both the amplitude and phase signatures of the Fano resonance. Our extensive hole-doping and magnetic field dependent investigation suggests that the Fano resonance may arise from an interplay between the superconducting fluctuations and the charge density wave fluctuations, prompting future studies to look more closely into their dynamical interactions.

The insertion of central venous catheters through the femoral veins is not uncommon and is potentially associated with the risk of immediate puncture-related complications and severe late complications as infection and thrombosis. As for other central venous access devices, the use of a standardized protocol of insertion and the correct application of evidence-based strategies are beneficial in reducing the risk of complications. We proposed a standardized protocol (S.I.F.: Safe Insertion of Femorally Inserted Central Catheters) consisting of seven strategies that should be part of vascular cannulation and should be adopted during the insertion of femoral venous catheters, aiming to minimize immediate, early and late insertion-related complications. These strategies include: preprocedural evaluation of the patient history and of the veins, appropriate aseptic technique, ultrasound guided puncture and cannulation of the vein, intra-procedural assessment of the tip position, adequate protection of the exit site, proper securement of the catheter, and appropriate coverage of the exit site.

Insertion of central venous catheters in the cervico-thoracic area is potentially associated with the risk of immediate/early untoward events, some of them negligible (repeated punctures), some relevant (accidental arterial puncture), and some severe (pneumothorax). Furthermore, different strategies adopted during insertion may reduce or increase the incidence of late catheter-related complications (infection, venous thrombosis, dislodgment). This paper describes a standardized protocol (S.I.C.: Safe Insertion of Centrally Inserted Central Catheters) for the systematic application of seven basic beneficial strategies to be adopted during insertion of central venous catheters in the cervico-thoracic region, aiming to minimize immediate, early, or late insertion-related complications. These strategies include: preprocedural evaluation, appropriate aseptic technique, ultrasound guided insertion, intra-procedural assessment of the tip position, adequate protection of the exit site, proper securement of the catheter, and adequate coverage of the exit site.

Phosphate shortages and the ensuing pressures on food security have led to an interest in processed sewage sludge as a substitute for commercial fertilisers. The presence of heavy metals in this nutrient source causes concerns around environmental release and pollution. This work builds towards a resin-in-pulp sludge detoxification process. It showcases the kinetic and thermodynamic adsorption capabilities of the ion-exchange resins C107E (carboxylic acid functionality), MTS9301 (iminodiacetic acid) and TP214 (thiourea), with respect to Cu(II), Fe(II), Pb(II) and Zn(II), within a simulated sewage sludge weak acid (acetate) leachate. The isotherms produced in this complex system were quite different to those generated when single metals were investigated in isolation, with desorption of lower affinity species clearly observed at higher equilibrium concentration values. Mixed-metal isotherm data were fitted to common two-parameter isotherm models and also a novel modified Langmuir model, which better accounted for the effects of desorption and competition. Kinetic data were also fit to common two-parameter models; results suggesting the system was likely film diffusion-controlled and followed pseudo-2nd-order kinetics. C107E displayed rapid adsorption of lead (t1/2 = 26 ± 3min), and significant uptake of all metals. MTS9301 showed high affinity for copper ions, with concurrent desorption of all the other metals, and also displayed the fastest kinetics (t1/2 = 14.1 ± 0.9, 130 ± 20, 25 ± 5 and 49 ± 6 min for copper, iron(II), lead and zinc, respectively). C107E and MTS9301 showed far slower adsorption for iron(II) than the other three metals, which invited the possibility of kinetic separations. TP214 had reasonable effectiveness in removal of copper, but poor affinity for all other metals. The greatest difficulty in modelling the multi-metal system was the two-stage trends observed in equilibrium experiments, as metal-proton exchanges become metal-metal exchanges. While not having the highest capacity, MTS9301 was recommended as the most appropriate resin for rapid and efficient removal of Cu, Pb and Zn from the acetate medium.

Insertion of Peripherally Inserted Central Catheters (PICCs) is potentially associated with the risk of immediate/early adverse events, some of them minimal (repeated punctures) and some relevant (accidental arterial puncture or nerve-related injury). Several strategies adopted during the insertion process may minimize the risk of such events, including late complication risks such as infection, venous thrombosis, or catheter dislodgment and/or malposition. This paper describes an update version of the SIP protocol (Safe Insertion of PICCs), an insertion bundle which includes eight effective strategies that aims to minimize immediate, early, or late insertion-associated complications. These strategies include: preprocedural ultrasound assessment utilizing the RaPeVA (Rapid Peripheral Venous Assessment) protocol; appropriate skin antiseptic technique; choice of appropriate vein, adoption of the Zone Insertion Method™; clear identification of the median nerve and brachial artery; ultrasound-guided puncture; ultrasound-guided tip navigation; intra-procedural assessment of tip location; correct securement of the catheter, and appropriate protection of the exit site. This updated version of the SIP protocol includes several novelties based on the most recent evidence-based scientific literature on PICC insertion, such as the clinical relevance of the tunneling technique, the use of ultrasound for intra-procedural tip navigation and tip location, and the new technologies for the protection of the exit site (cyanoacrylate glue) and for the securement of the catheter (subcutaneous anchorage).

The aim of this report is to present the root cause analysis on failed patient-specific quality assurance (QA) measurements of pencil beam scanning (PBS) protons; referred to as PBS-QA measurement. A criterion to fail a PBS-QA measurement is having a <95% passing rate in a 3.0%-3.0 mm gamma index analysis. Clinically, we use a two-dimensional (2D) gamma index analysis to obtain the passing rate. The IBA MatriXX PT 2D detection array with finite size ionization chamber was utilized. A total of 2488 measurements performed in our PBS beamline were cataloged. The percentage of measurements for the sites of head/neck, breast, prostate, and other are 53.3%, 22.7%, 10.5%, and 13.5%, respectively. The measurements with a passing rate of 100 to >94%, 94 to >88%, and <88% were 93.6%, 5.6%, and 0.8%, respectively. The percentage of failed measurements with a <95% passing rate was 10.9%. After removed the user errors of either re-measurement or re-analysis, 8.1% became acceptable. We observed a feature of >3% per mm dose gradient with respect to depth on the failed measurements. We utilized a 2D/three-dimensional (3D) gamma index analysis toolkit to investigate the effect of depth dose gradient. By utilizing this 3D toolkit, 43.1% of the failed measurements were improved. A feature among measurements that remained sub-optimal after re-analysis was a sharp >3% per mm lateral dose gradient that may not be well handled using the detector size of 5.0 mm in-diameter. An analysis of the sampling of finite size detectors using one-dimensional (1D) error function showed a large dose deviation at locations of low-dose areas between two high-dose plateaus. User error, large depth dose gradient, and the effect of detector size are identified as root causes. With the mitigation of the root causes, the goals of patient-specific QA, specifically detecting actual deviation of beam delivery or identifying limitations of the dose calculation algorithm of the treatment planning system, can be directly related to failure of the PBS-QA measurements.

Functionalized hypercrosslinked polymers (HCPs) with surface areas between 213 and 1124 m2/g based on a range of monomers containing different chemical moieties were evaluated for CO2 capture using a pressure swing adsorption (PSA) methodology under humid conditions and elevated temperatures. The networks demonstrated rapid CO2 uptake reaching maximum uptakes in under 60 s. The most promising networks demonstrating the best selectivity and highest uptakes were applied to a pressure swing setup using simulated flue gas streams. The carbazole, triphenylmethanol and triphenylamine networks were found to be capable of converting a dilute CO2 stream (>20%) into a concentrated stream (>85%) after only two pressure swing cycles from 20 bar (adsorption) to 1 bar (desorption). This work demonstrates the ease with which readily synthesized functional porous materials can be successfully applied to a pressure swing methodology and used to separate CO2 from N2 from industrially applicable simulated gas streams under more realistic conditions.

We introduce fluoride-selective anion exchange resin sorbents as sensitisers into membranes for water-gated field effect transistors (WGTFTs). Sorbents were prepared via metal (La or Al)-loading of a commercial macroporous aminophosphonic acid resin, PurometTM MTS9501, and were filled into a plasticised poly(vinyl chloride) (PVC) phase transfer membrane. We found a potentiometric response (membrane potential leading to WGTFT threshold shift) to fluoride following a Langmuir-Freundlich (LF) adsorption isotherm with saturated membrane potential up to ~480 mV, extremely low characteristic concentration c1/2 = 1/K, and picomolar limit of detection (LoD), even though ion exchange did not build up charge on the resin. La-loading gave a superior response compared to Al-loading. Membrane potential characteristics were distinctly different from charge accumulating sensitisers (e.g., organic macrocycles) but similar to the Cs+ (cation) selective ion-exchanging zeolite mineral 'mordenite'. We propose a mechanism for the observed threshold shift and investigate interference from co-solutes. Strong interference from carbonate was brought under control by 'diluting' metal loading in the resin. This work sets a template for future studies using an entirely new 'family' of sensitisers in applications where very low limit of detection is essential such as for ions of arsenic, mercury, copper, palladium, and gold.

A dispersible porous polymer (PEG113 -b-DVB800 -co-AA200 ) based on the controlled radical polymerization of divinylbenzene and acrylic acid with a poly(ethylene glycol) (PEG) macrochain transfer agent (macro-CTA) is synthesized and postsynthetically modified with anthracene. This blue-emitting porous polymer is used to encapsulate the yellow-emitting fluorophore rhodamine B into its core, resulting in a white-light emitting dispersion with a quantum yield of 38% and commission internationale de l'éclairage coordinates of (X = 0.33, Y = 0.32).

MXenes are a recently discovered class of two-dimensional materials that have shown great potential as electrodes in electrochemical energy storage devices. Despite their promise in this area, MXenes can still suffer limitations in the form of restricted ion accessibility between the closely spaced multistacked MXene layers causing low capacities and poor cycle life. Pillaring, where a secondary species is inserted between layers, has been used to increase interlayer spacings in clays with great success but has had limited application in MXenes. We report a new amine-assisted pillaring methodology that successfully intercalates silica-based pillars between Ti3C2 layers. Using this technique, the interlayer spacing can be controlled with the choice of amine and calcination temperature, up to a maximum of 3.2 nm, the largest interlayer spacing reported for an MXene. Another effect of the pillaring is a dramatic increase in surface area, achieving BET surface areas of 235 m2 g-1, a sixty-fold increase over the unpillared material and the highest reported for MXenes using an intercalation-based method. The intercalation mechanism was revealed by different characterization techniques, allowing the surface chemistry to be optimized for the pillaring process. The porous MXene was tested for Na-ion battery applications and showed superior capacity, rate capability and remarkable stability compared with those of the nonpillared materials, retaining 98.5% capacity between the 50th and 100th cycles. These results demonstrate the applicability and promise of pillaring techniques applied to MXenes providing a new approach to optimizing their properties for a range of applications, including energy storage, conversion, catalysis, and gas separations.

In high-energy physics, the Higgs field couples to gauge bosons and fermions and gives mass to their elementary excitations. Experimentally, such couplings can be inferred from the decay product of the Higgs boson, i.e., the scalar (amplitude) excitation of the Higgs field. In superconductors, Cooper pairs bear a close analogy to the Higgs field. Interaction between the Cooper pairs and other degrees of freedom provides dissipation channels for the amplitude mode, which may reveal important information about the microscopic pairing mechanism. To this end, we investigate the Higgs (amplitude) mode of several cuprate thin films using phase-resolved terahertz third harmonic generation (THG). In addition to the heavily damped Higgs mode itself, we observe a universal jump in the phase of the driven Higgs oscillation as well as a non-vanishing THG above Tc. These findings indicate coupling of the Higgs mode to other collective modes and potentially a nonzero pairing amplitude above Tc.

Sulfonated hyper-cross-linked polymers based on 4,4'-bis(chloromethyl)-1,1'-biphenyl (BCMBP) were synthesized via metal-free (SHCP-1) and conventional Lewis acid-catalyzed (SHCP-2) Friedel-Crafts alkylation routes. The sulfonated polymers possessed BET surface areas in excess of 500 m2·g-1. SHCP-1 was investigated for its ability to extract Sr and Cs ions from aqueous solutions via the ion-exchange reaction of the sulfonic acid moiety. Equilibrium uptake data could be accurately modeled by the Dubinin-Radushkevich isotherm, with maximum calculated loading values of 95.6 ± 2.8 mg·g-1 (Sr) and 273 ± 37 mg·g-1 (Cs). Uptake of both target ions was rapid, with pseudo second-order rate constants calculated as 7.71 ± 1.1 (×10-2) for Sr and 0.113 ± 0.014 for Cs. Furthermore, the polymer was found to be highly selective toward the target ions over large excesses of naturally occurring competing metal ions Na, K, Mg, and Ca. We conclude that hyper-cross-linked polymers may offer intrinsic advantages over other adsorbents for the remediation of aqueous Sr and Cs contamination.

A shortage of phosphate rock resources has led to a focus on the use of sewage sludge as a fertiliser. One factor preventing the unlimited application of sewage sludge in agriculture is the heavy metal content. This work looks at a process where weak acid leaching of sewage sludge would be coupled to ion exchange (IX) for heavy metal ion removal. IX offers an effective method for the recovery of these metals from a leachate. This study presents the pH performance of six selected IX resins in extracting Cu2+, Fe2+, Pb2+ and Zn2+ from acetic, lactic and citric acid media simulated weak acid leachate. Acetic acid media displayed limited suppression of metal extraction, whilst lactic and citric acid media rendered MTS9100 ineffective. Lactic acid media when combined with C107E resin allows for the targeted extraction of lead. Both MTS9570 and MTS9501 display high ferrous extraction in all media at all pH values, although citric acid hinders extraction by both resins at higher pH values. TP214 is selective for copper over all other metal ions in all weak acid media. MTS9301 is the most effective at extracting all metals from all media solutions, with separation of copper becoming more apparent when transitioning to citric acid.

PURPOSE: To evaluate the feasibility and accuracy of intracavitary electrocardiogram for verifying tip position of peripherally inserted central catheters in cancer patients during follow-up period. METHODS: From March 2015 to October 2015, 126 patients involved in eight hospitals who underwent peripherally inserted central catheter placement received intracavitary electrocardiogram and chest X-ray to verify position of the catheter tip during follow-up period. Their intracavitary electrocardiogram was compared with surface electrocardiogram to judge catheter tip landing zone in one of three different anatomical zones. The amplitude of intracavitary electrocardiogram P wave and the ratio of intracavitary electrocardiogram P wave/surface electrocardiogram P wave were measured and showed correlation with catheter tip position confirmed by chest X-ray. Based on chest X-ray principle, all the cases were assigned into three intracavitary electrocardiogram groups to explore the optimal cut-off values for intracavitary electrocardiogram P wave and intracavitary electrocardiogram P wave/surface electrocardiogram P wave by analyzing the receiver operating characteristic. RESULTS: No technique-related complications or adverse events occurred in this study. The matching rate between intracavitary electrocardiogram and chest X-ray method was 93.7%. The optimal cut-off values for intracavitary electrocardiogram P wave were set from 3.15 to 3.75 mV, and intracavitary electrocardiogram P wave/surface electrocardiogram P wave from 1.65 to 3.25. CONCLUSIONS: It is demonstrated in this retrospective multicenter study that the intracavitary electrocardiogram method for verifying tip position of peripherally inserted central catheter during follow-up period is feasible and accurate in all adult patients with cancer.

RESUMO

Two M8L12 cubic coordination cages, as desolvated crystalline powders, preferentially adsorb CO2 over N2 with ideal selectivity CO2/N2 constants of 49 and 30 at 298 K. A binding site for CO2 is suggested by crystallographic location of CS2 within the cage cavity at an electropositive hydrogen-bond donor site, potentially explaining the high CO2/N2 selectivity compared to other materials with this level of porosity.
